Authorities said to be investigating possible institutional neglect in case of HR McMaster Sr.

The father of former National Security Adviser HR McMaster died April 13, and sources tell 6ABC the death has been labeled as suspicious. HR McMaster Sr., 84, died at Cathedral Village, a senior living facility in Philadelphia's Roxborough neighborhood where he'd been receiving care after having a stroke. He reportedly fell and hit his head and was placed in a recliner, but he didn't receive treatment or monitoring, per NBC10's sources; he later died. His cause of death was found to be blunt impact trauma to the head, and the death was ruled an accident. Sources say some staff members told the family records about the death were falsified and that the family had been given misleading information. CBS3 reports that authorities are investigating the possibility of institutional neglect.

The Philadelphia Police Department Homicide Unit, the Philadelphia District Attorney's Office, the Pennsylvania Attorney General's Office, and the Department of Health are all said to be investigating. Cathedral Village says it's cooperating with the investigation and completing its own internal investigation. (More HR McMaster stories.)
